Dana Sandrik, FNP - C

Dana Sandrik, FNP-C, is the Founder and Clinical Director of Lifeline Medspa and a highly trained Family Nurse Practitioner with more than seven years of focused experience in evidence-based weight-loss management and aesthetic medicine. Dana believes in a comprehensive, patient-centered approach that integrates medical science, nutrition expertise, and advanced aesthetic techniques to help individuals look and feel their best.

Dana earned her Master of Science in Nursing (MSN) from Chamberlain University and her Bachelor of Science in Human Nutrition from Arizona State University (ASU) — a combination that uniquely positions her as both a clinical practitioner and a metabolic health specialist. Her foundation in nutrition science allows her to design personalized, results-driven treatment plans rooted in physiology, behavior change, and sustainable lifestyle practices.

She began her career as a Registered Nurse at Banner Thunderbird, where she gained four years of hands-on clinical experience while completing her graduate studies. After becoming a board-certified Family Nurse Practitioner, Dana spent three years at District Medical Group (DMG) in family practice with a concentration in medical weight loss. During this time, she refined her expertise in metabolic health, chronic disease prevention, patient education, and long-term weight-management protocols.

Dana is deeply passionate about aesthetic medicine, with a strong interest in facial rejuvenation, injectables, and treatment plans that enhance natural beauty. She is committed to continual professional development and has pursued advanced training to offer safe, modern, and effective aesthetic treatments. Her philosophy blends clinical precision with artistic vision, helping patients achieve confidence through results that look refined, balanced, and natural.

Daryl Putman, BSN, RN

Hi everyone! My name is Daryl Putman RN, BSN. I am so excited to not only be working with my sister Dana again, but this time for her, at Lifeline Weightloss and Medspa. I will be working as a manager and Aesthetic Nurse.

I was born and raised in Peoria, Arizona. I have been working in the medical field for more than 10 years. Through my experiences as an Acute Care Nurse, Medical Assistant, and Office Manager for a Weight loss and Family Practice, I have developed a deep understanding and appreciation of the benefits of having a healthy and active lifestyle. I am raising three beautiful, caring and energetic boys and you can find us anywhere outside- swimming, camping, or riding bikes. Exploring the outdoors is one thing that keeps myself and my sons thriving in our busy schedule. I am excited to bring my knowledge and passion for healthy living to Lifeline as a Clinical assistant.

Throughout the past year, I have changed my career path from Acute Nursing to Aesthetic Nursing. I am focused on learning and developing my aesthetic skills so that I am able to provide our clients with a natural looking result as well as the highest quality skin care. I want everyone to leave our office looking and feeling their best!

Shaunte Sandrik, BSN, RN

I’m Shaunte, a nurse injector At Lifeline Medspa. Previous to aesthetic nursing, I have spent the last 9 years working in the Emergency Department, with the last 2.5 years being in the Pediatric Emergency Department as an RN.

Aesthetics have always been a passion of mine! I obtained my certification in neurotoxins and filler from Empire Medical Training. Health and wellness has been a longstanding passion of mine and I am beyond excited to focus my time on aesthetics and wellness. I aim to deliver natural and youthful results that are tailored to your goals.

When I’m not working, you can find me with my family. I have two beautiful girls, a wonderful husband, a hamster named Cupcake and a dog named Nala. We love to be outside riding anything with two wheels and spending time with the rest of our family.

Hi! I’m Phoenix! In 2024 I obtained my certification as a licensed medical assistant from WestMec. My goal is to continue going to school for nursing and hopefully go into pediatrics or aesthetic nursing. I’m so excited for the opportunity I have received to work here and I can’t wait to help you!
